| The primary objective of MSSP is to avoid or delay the inappropriate placement
of persons in nursing facilities, while fostering independent living in the community.
MSSP provides services to eligible clients and their families to enable clients to remain
in their homes. MSSP works within a social and medical model to provide a coordination of
services required to maintain client independence. MSSP provides the following: |
- Assist seniors in identifying and accessing programs and care services such as meal preparation or home-delivered meals, emergency alert devices,
assistance with government or insurance paperwork, transportation, nursing care, legal advice,
low-cost housing, and much more.
- Provide detailed care planning, identifying independence goals and objectives.
- Provide intense case management of clients' involvement in programs and systems.
- Plans and coordinates the use of public and private programs and services for clients.
- Purchase of client services when required.
- Report detailed program activity information to State and Federal agencies.
